Risk Management Form

Risk Management Form

Each year, every Coach, Board Member, Referee and anyone else involved with children, must submit to have a Risk Management and background check done. Please follow the link below.

 

OSA/GotSport Risk Instructions

  • Log into GotSport(if you don’t know your password, select “Forgot Password?”)
  • From your profile, add a photo if required
  • Reconfirm your DOB is correct
  • PC Users:Click on Dashboardin the upper left corner to access your OSA risk
  • Mobile Device Users:Click the green 3-bar menu to select Dashboardand access your OSA risk
  • Click on Details, next to each OSA risk requirement that is Requiredor Expiredfor the applicable registration year for instructions on how to successfully complete

Please note that no one risk requirement can expire during the middle of the seasonal year.
